Services Registry - Services
Our program provides the consumer with services that include:
* Telephone interview
* Home visit by professional coordinator of the Registry at which time
a needs assessment is made
* Consumer's applicable family members and coordinator of the Registry
reach consensus as to the anticipated skills required by the home care
provider
*Appropriate list of workers is referred to the consumer with a minimum
of three prescreened names of individuals
* Within 14 days, a follow up call is made to determine if a successful
match has occurred. If not, the process continues with an additional list
of prospective workers sent to the consumer.
