COVID-19 update: Fall prevention services (environmental assessment, medication review, exercise, education) are currently being delivered virtually (via Zoom, FaceTime, GrandPad; or outside visits) and we are coordinating minor home modifications for those who qualify.

About the Program
This is one of the few fall prevention programs nationwide that can come help you in your home- where most falls happen!
Falls very often lead to institutionalization and complex health problems or dependence on others. Our program helps you with a strength and balancing routine that fits you. We can also do a home safety check with you, provide minor home modifications and a medication review for possible drug interactions which are a significant risk factor in falls.

This program is funded by County Measure A via the Alameda Area Agency on Aging and the Alameda County Board of Supervisors and generous individuals, organizations, and businesses in the community.
